Histological diagnosis: Foci encephalomalaciae multiplices cerebri et trunci. Arterio et arterio-losclerosis gradu mango. Autopsy examination of 73-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with cresyl-violet, Heidenhain, Holzer and Van Gieson's method.Large foci of malacia were observed in specimens of the right frontal region, right occipital region and right thalamus. They all had a similar nature of advanced dissection with partial clearing of disintegrated tissue, abundant macrophage reaction, and a rich capillary network. Gliosis with gemistocytic reaction was present in the adjacent tissue. Fibrous gliosis surrounded only the focus in the occipital region and in the thalamus. Numerous hollow and perivascular lacunae, without macrophages were observed in the basal nuclei. They were surrounded by fibrous gliosis and starchy bodies. Subependymal gliosis was also intense. The described focus of malacia in the occipital region was separated from the ventricular wall by a fibrous gliosis. Partial disintegration of the substancia nigra was observed. Larger caliber arteries had atherosclerotic plaques, small and arterioles were often vitreous altered, all characterized by thick walls. Fragmentation of the elastic membrane was evident.
