Description:

Clinical, anatomical and histological diagnosis

Abstract:

Histological diagnosis: Encephalomalacia recens rubra. Oedema cerebri. Foci haemorrhagiae sec. pontis. Autopsy examination of 69-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with H-E and Van Gieson's method. In the right frontal lobe, the edge of a hemorrhagic focus of fresh malacia was visible. Numerous circular and band-like hemorrhages around vessels were observed, especially in the cortex, among which intense leukocytic infiltration in the brain tissue was found in places. Besides, glial activation, endothelial swelling and symptomatic infiltrates around vessels were found. Nerve cells were severely damaged, mostly with ischemic changes, shrunken, with fatty degeneration. Extensive cell loss obliterating the layered structure of the cortex was also observed. The brain as a whole showed features of severe edema. The cerebral vessels had small fibrous changes. Fresh blood, single leukocytes, macrophages and symptomatic infiltrates were present in the subarachnoid space. The meninges revealed features of fibrosis. There were scattered fresh hemorrhagic foci in the pons. Around the foci, weaker staining of nerve cells, marked glial excitation, features of congestion and edema were observed
