Abstrakt:

Histological diagnosis: Encephalomalacia cum haemorrhagia recidivans in regione gyrorum orbitalium. Aneurysma arteriae cerebri anterioris. Autopsy examination of 43-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain sections stained with Hematoxylin-eosin and van Gieson and aneurysm paraffin sections stain with Mallory stain and Masson trichrome.The specimen showed several foci, or multiple parts of one larger focus of malacia. Numerous macrophages were visible in the focus, peripherally hypertrophied glia. In the largest focus of malacia, hemorrhage was present, part of which was composed of unaltered fresh blood cells and part of which was composed of disintegrated, with hemosiderin-filled macrophages in their surroundings. The cortical cells showed defects and ischemic changes. Edema and symptomatic infiltrates with predominance of small, round cells were seen around the vessels. In the meninges, the infiltrates consisted mostly of lymphocyte-like cells, and there were numerous macrophages with hemosiderin near the hemorrhage. The aneurysm preparation did not cover the entire vessel circumference. It had a heterogeneous and irregular wall, very thickened in places and thinned in places. The wall was predominantly composed of connective tissue, with muscular fibers visible in places.
