Abstract:

The aim of the study was to assess whether there are some disorders of the hypothalamic-pituitary-adrenal axis in patients with type 1 diabetes and recurrent hypoglycemia. Additionalgoals included the comparison of metabolic control between the group of patients with episodichypoglycemia and patients with recurrent hypoglycemia, evaluation of the presence ofantibodies characteristic for some autoimmune diseases not related to the hypothalamic-pituitary-adrenal axis, and the determination of whether the medical history remains sufficientlysensitive to detect hypoglycemia in comparison with objective methods such as continuousblood glucose monitoring (CGM) and blood glucose profiles during hospitalization. Conclusions1. It seems that hypocortisolemia is an unlikely cause of hypoglycemia in patients withtype 1 diabetes. Routine diagnosis of hypocortisolemia in patients with recurrenthypoglycemia is not necessary unless there are other reasons for it.2. Diabetes control assessed by HbA1c in the group of patients with episodic andrecurrent hypoglycemia based on history and CGMs was similar, while patients withrecurrent hypoglycemia in hospital conditions have a higher percentage of HbA1c%compared to patients with episodic hypoglycemia during hospitalization.3. The prevalence of antibodies characteristic of some autoimmune diseases not relatedto the pituitary-adrenal axis in patients with type 1 diabetes with episodic andrecurrent hypoglycemia is similar. Therefore, it seems that these diseases don’tinfluence the frequency of hypoglycemia in these patients.4. The sensitivity of the medical history in detecting hypoglycemia is low. The objectivemethods, such as CGM is needed to evaluate the prevalence of hypoglycemia inpatients with type 1 diabetes.
