Arteriosclerosis. Autopsy examination of 66-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with H ematoxylin-eosin and V an Gieson's method.In the left parietal lobe, a foci of malacia in the macrophage clearing phase was found in the white matter, extending to the edge of a fresh hemorrhagic focus. The surrounding tissue contained numerous macrophages and proliferating vessels.Large swelling of white matter, endothelial cells, glial cells, and stimulation of astroglia were observed around this focus. Additionally, loosening of the tissue structure, corresponding to the initial phase of edematous necrosis, was seen in some areas. There were also several smaller foci of necrosis scattered in white matter subcortically with the same morphology as in the main focus, but without hemorrhagic component. In places, clusters of leukocytes, macrophages with hemosiderin, or small cells symptomatic infiltrates were found near the vessels. Atrophy of neurons in the cortex, complete loss of layer structure in some places, ischemic cell disorders and fatty degeneration was found. In the meninges small symptomatic infiltrations were seen.
